Confirm your eligibility with your Regional Center

To be eligible for SDP, you must be an active client of your local regional center that is funding your services. Your developmental disability should also qualify under Welfare and Institutions Code 4512. Children under 3 years of age are eligible if they meet requirements under the Lanterman Act and the Welfare and Institutions Code 4512. Please follow up with your regional center to ensure your eligibility with SDP.

Contact your Regional Center Service Coordinator / Social Worker / Case Manager

If you have interest in joining SDP, contact your primary regional center assigned person which is called a service coordinator or case manager or social worker, depending on the regional center. They will help you at different points along your transition. For example, they will direct you to mandatory orientations, IPP meetings, and confirm your eligibility.

Attend the Required SDP Orientation

The attendance at the two session SDP Orientation is mandatory.  The orientation provides an overview of SDP, including participant rights and responsibilities, budgeting, and how to work with vendors and support staff.

Submit SDP Orientation Certificates of completion to initiate the SDP Application

Once you have completed both sessions in the mandatory Self-Determination Program orientation, you will receive certificates of completion, one for each part. Submit those certificates to your regional center service coordinator / case manager / social worker to begin enrollment. This will indicate your interest in SDP and readiness to start the next step with them.
